Role Summary
We are seeking a dedicated and compassionate Certified Nursing Assistant to join our clinical team on an immediate hire basis. In this role, you will provide direct, hands-on care to patients in their homes, supporting their comfort, dignity, and quality of life. Key Responsibilities
- Provide personal care assistance to patients, including bathing, grooming, dressing, and toileting
- Assist patients with mobility, transfers, and positioning
- Monitor and record vital signs and report changes in patient condition to the appropriate clinical staff
- Support patients with meal preparation and feeding as needed
- Maintain a clean, safe, and comfortable environment for patients
- Follow individualized care plans developed by the clinical team
- Communicate effectively with patients, families, and other members of the care team
- Document care provided in a timely and accurate manner
- Adhere to all infection control, safety, and confidentiality standards
Qualifications and Requirements
- Current Certified Nursing Assistant (CNA) certification in good standing
- Prior experience in home health, hospice, or a similar caregiving setting preferred
- Strong communication and interpersonal skills
- Ability to work independently and manage time effectively while traveling between patient homes
- Reliable transportation and a valid driver's license
- Compassionate, patient, and respectful demeanor
- Ability to follow care plans and clinical instructions accurately
- Comfortable working with patients of varying ages, backgrounds, and health conditions
